Wellness to provide Nutrition Counselling Sessions to myself or the client for which I am legally
responsible. The consult(s) will provide information and guidance about health factors within my
own control: my diet, nutrition, and lifestyle. For group courses such as The Food Freedom Project - i am legally responsible for all information and actions i apply to my routine

 I understand that Brittany Lynn McCann is a holistic practitioner -not a medical physician- and
does not dispense medical advice, nor will she diagnose or treat any medical condition, but will
provide nutritional support and nutrition education for an already diagnosed condition

 She provides education to enhance my knowledge of health through the use of whole foods,
dietary supplements, and emotional awareness. While nutritional and botanical support can be an
important compliment to my medical care, I understand these services are not a substitute for
medical care. I will discuss with my doctor if I want to come off medication
 I acknowledge Brittany provides education around my emotional body, using somatic release
techniques. I would always let her know if I were in a vulnerable state, and reach for professional
help
 Medical records and personal information and history divulged in session to Brittany Lynn Wellness will be kept confidential, unless I consent to sharing my medical information.             

It is important that I tell my nutritional therapist about any medical diagnosis, medication, herbal medicine, or food supplements, I am taking as this may affect the nutritional programme.            

 If I am unclear about the agreed nutritional therapy program/food supplement doses/time period, I should contact my nutritional therapist promptly for clarification 

I must contact my nutritional therapist should I wish to continue any specified supplement programme for longer than the original agreed period, to avoid any potential adverse reactions.

Recording  ( group) classes via zoom ( permission required before entering zoom room )using any form of electronic media is not allowed without the checked box- of both me and my Nutritional Therapist.

If used in an online portal space such as The Food Freedom Project -I will let Brittany know if i am uncomfortable with this option.

 

I agree to Brittany McCann of Brittany Lynn Wellness being harmless for claims or damages in connection with
our work together. This is a contract between myself and Brittany McCann, of Brittany Lynn Wellness and I understand that it is also a release of potential liability.
